Information We Collect And How We Use It
We may collect the following types of information:
- Information you provide – When you sign up for an account with us, we ask you for personal information and personal information and health information for the senior for whom you are caring including but not limited to name, e-mail address, and the senior’s medical information. We may combine the information you provide us with information from third parties to provide you with a better experience and to improve the quality of our services.
- Cookies – When you visit our website, we send one or more cookies to the device from which you've visited our site. We use cookies to improve the quality of our service, for storing user preferences and tracking user trends.
- Log Information – When you visit our site, our servers automatically record certain technical information about your visit, such as the URL you requested, your interaction with a service at that URL, your internet protocol address, your browser type, browser language, the date and time of your request, and the information contained in on or more cookies that may uniquely identify your browser or your account.
- User communications – If you use our website to communicate with us, we may retain those communications to process your inquiries, respond to your requests and improve our services.
- To send you newsletters- We will use your email address to send newsletters to you. Out of respect for your privacy, you can unsubscribe by clicking the “Unsubscribe” link at the bottom of each e-mail.
- Medical Information – We may collect medical information about you or the senior for whom you are caring (including but not limited to diagnoses, allergies, and prescriptions) to provide you services. In addition to the policies stated here, any medical information we collect is protected by our HIPAA Privacy Notice.
- PII – PII may be used to provide account administration, administer surveys, and respond to your questions or concerns.
In addition to the uses of specific information described above, we may use information you provide us to provide, maintain, protect, or improve our services, and develop new services; or to protect our rights or property or the rights or property of our users. If we use your information in a manner different than the purpose for which it was collected, we will ask for your consent prior.

You can log in to our site using sign-in services such as Facebook Connect. These services will authenticate your identity and provide you the option to share certain personal information with us such as your name and email address to pre-populate our sign up form. SMS Messaging
If applicable, this website, www.CareFamily.com may make available a service either directly or through third parties by which you can receive messages on your wireless device via short message service ("SMS Service"). Your provider's standard data and messaging rates apply to all SMS correspondence. All charges are billed by and payable to your mobile service provider. You represent that you are 18 years of age and the owner or authorized user of the wireless device on which messages will be received, and that you are authorized to approve the applicable charges. Data obtained from you in connection with this SMS Service may include your name, address, cell phone number, your provider's name, and the date, time, and content of your messages. The use of this information will be in accordance with this Policy. If fees are charged to your wireless account invoice, we may provide your carrier with your applicable information in connection therewith. CareFamily Operations

Your Rights With Respect To Your Medical Information
Right to inspect and copy your records
You have the right to review and to get a copy of medical information that may be used to make decisions about your care. This includes general medical records, but does not include notes on psychotherapy sessions. We have the right in some circumstances to deny your right to review or copy your records, but if we do so, you have the right to have our decision reviewed. We may charge you a fee for the cost of copyright, mailing or other supplies required for us to comply with your request.
Right to amend your information
You have the right to request that we amend any medical information we have about you. However, we will not amend information that we did not create or that is not part of the medical information we have about you, that is not part of information you would be allowed to inspect or to copy, or that we determine is accurate and complete. If you wish us to amend your information, please contact us and provide a reason for the amendment.
